Information requested by the forms

• Peptide guide: your email address and selected language for the guide request.
• Peptides contact request: first name, email, state, optional phone (required for a phone reply), language, contact preferences and any multiple-choice answers you complete about goals, priorities, timing, next step and availability of recent labs.
• Beauty & Longevity opening updates: first name, email address, selected interest and language.

How requests are used

When the connection is enabled, the forms send submitted details to POLI’s HighLevel system to record the request, organize follow-up and respond or send the requested material. The request type, date, language and submission-notice version are recorded. A guide request does not include a marketing subscription. Optional Peptides marketing consent is recorded separately.

If a form states that it is a preview and does not send data, its answers stay in that page. The completion message states whether a request was sent. A contact request does not reserve clinical care.

Calendars and website providers

The website is hosted on Vercel. Embedded calendars and their external links use LeadConnector/HighLevel. Opening a calendar loads content from that provider; details entered there are submitted through its form. The current calendars are demonstrations: use test details only, without medical information or payments.

Preferences in your browser

The website saves your language preference in browser local storage when available. In-progress answers stay in page memory; refreshing may clear them. The contact form does not add health goals or answers to web addresses.

Medical information and the final policy

Do not send medical records, medicines, lab results or clinical files through these forms or test calendars. The website does not provide a secure medical-document upload. POLI must confirm the clinical channel and applicable privacy policy before requesting those details.
